Ashley Vazquez currently accepts Blue Cross Blue Shield, Harvard Pilgrim, Aetna, Unicare, and MGH (AllWays) insurance. If you have another insurance company, you may be  eligible of out-of-network services. Co-payment and/or full payment is due at time of service. Cash, check, and credit cards accepted. A superbill (medical receipt) can be provided that may be submitted to your insurance provider for possible full or partial reimbursement based on your insurance coverage. Submitting a superbill with the insurance company is the sole responsibility of the individual and there is no guarantee of reimbursement.

Here is a list of questions to ask your insurance provider to better understand if out-of-network services may be covered:

  • Do you cover nutrition sessions?  If so, do you cover out-of-network dietitians?

  • If you cover out-of-network nutrition providers, how many sessions are covered per year?

  • Are there any medical diseases or diagnoses you cover?

  • What percentage of my nutrition bill is covered by insurance?

  • Do I need approval from my Primary Care Physician in order to receive nutrition coverage?
